What Is Limb Lengthening Surgery?

Limb lengthening surgery is a procedure in which an orthopedic surgeon makes a controlled cut in a bone and gradually separates the two bone segments.

The body responds to the small space created between the bone segments by producing new bone.

This biological process is known as distraction osteogenesis.

Rather than creating the entire desired length during surgery, the distance between the bones is increased very gradually over a period of weeks or months. This gives bone, muscles, nerves, blood vessels, tendons and other surrounding tissues time to adapt.

Once the desired length has been reached, lengthening stops and the newly formed bone is allowed to become stronger and harder.

The final result is not an artificial extension placed between two bones. The gap eventually fills with the patient's own regenerated bone.

How Does Limb Lengthening Work?

Bone has a natural ability to repair itself after a fracture or controlled surgical cut.

Limb lengthening uses this ability in a carefully controlled way.

During surgery, an osteotomy is performed. This means the surgeon creates a controlled separation in the bone that will be lengthened.

A specialized lengthening device then stabilizes the bone.

After a short waiting period following surgery, known as the latency period, gradual lengthening begins.

The two sections of bone are slowly moved apart, generally by fractions of a millimeter several times during the day.

As the distance increases, new bone develops inside the gap.

The goal is to lengthen slowly enough for new bone to form while also giving the surrounding soft tissues time to adapt.

For this reason, the speed of lengthening is carefully monitored and may be adjusted according to X-rays, flexibility, nerve symptoms, pain and bone formation.

Why Is Limb Lengthening Surgery Performed?

Limb lengthening was originally developed primarily as reconstructive orthopedic surgery.

It continues to be used for medical conditions such as:

• Leg length discrepancy

• Congenital limb shortening

• Bone growth disorders

• Skeletal dysplasia

• Deformities of the femur or tibia

• Bone loss following trauma

• Certain complications following fractures or previous surgery

The same biological principle can also be used for cosmetic stature lengthening, where a person with normally developed but shorter legs chooses surgery specifically to increase height.

Medical limb reconstruction and cosmetic height lengthening use many of the same orthopedic principles, but patient selection, planning and treatment goals may be different.

Which Bones Can Be Lengthened?

Several bones in the body can be lengthened when medically appropriate.

For cosmetic height increase, the two primary bones are the:

Femur: the thigh bone between the hip and knee.

Tibia: the main lower-leg bone between the knee and ankle.

The humerus in the upper arm can also be lengthened in selected medical or cosmetic cases.

For height increase, surgery is normally performed on both legs so that the body remains symmetrical.

Femur Lengthening vs Tibia Lengthening

Both the femur and tibia can contribute to height, but they have different anatomy and rehabilitation considerations.

Femur Lengthening

The femur is the longest bone in the human body and is commonly chosen for cosmetic stature lengthening.

Femoral lengthening requires the thigh muscles and other soft tissues surrounding the hip and knee to adapt progressively during distraction.

Maintaining hip and knee mobility is therefore an important part of rehabilitation.

Tibia Lengthening

Tibial lengthening involves the lower leg.

Because the tibia is connected functionally with the fibula, the fibula must also be appropriately managed during tibial lengthening.

Calf muscles, the Achilles tendon, knee movement and ankle mobility require careful monitoring throughout the process.

Some patients undergo only femoral or only tibial lengthening. Others may undergo lengthening of both segments as part of a larger treatment plan.

The correct choice depends on anatomy, proportions, medical condition, flexibility, bone quality and the amount of additional height being considered.

How Much Taller Can Limb Lengthening Surgery Make You?

There is no single appropriate lengthening amount for every patient.

The possible increase depends on factors such as:

• Which bone is being lengthened

• Starting bone length

• Body proportions

• Muscle flexibility

• Nerve tolerance

• Bone formation

• Joint mobility

• Patient age and general health

• Type of implant

• Surgeon assessment

• Response during the lengthening process

In cosmetic femoral lengthening, approximately 5 to 8 cm is a commonly discussed range.

Tibial lengthening is generally planned more conservatively, commonly around 4 to 6 cm.

At Lengthening Turkey, our maximum planned lengthening targets for cosmetic cases are generally:

Femur: up to 8 cm

Tibia: up to 6 cm

When both segments are lengthened, a total increase of approximately 12 to 14 cm may be considered in carefully selected patients.

These numbers should not be interpreted as guaranteed targets.

A planned lengthening may need to be slowed or stopped earlier if the patient's muscles, nerves, joints or bone regeneration indicate that continuing would create unnecessary risk.

In limb lengthening, achieving the largest possible number of centimeters should never take priority over function, alignment and safety.

The Stages of Limb Lengthening Surgery

Limb lengthening is best understood as a process rather than a single operation.

1. Preoperative Planning

Before surgery, the orthopedic team evaluates the patient's bones, joints, proportions, alignment, flexibility and general medical condition.

Full-length standing X-rays can be particularly important because they allow the surgeon to examine the mechanical axis of the legs.

This helps identify existing bowing, knock knees, rotational problems or differences in alignment before lengthening begins.

2. Surgery and Osteotomy

During surgery, the selected lengthening device is inserted or attached and the bone is carefully divided.

For an internal system, an intramedullary nail is positioned inside the bone.

For techniques involving external fixation, pins or screws connect the bone to a device outside the leg.

The bone remains stabilized despite the controlled osteotomy.

3. Latency Period

Lengthening does not normally begin immediately after surgery.

A short period allows the early healing process to begin around the osteotomy.

The exact timing is determined by the surgeon.

In many protocols, distraction begins approximately one week after surgery.

4. Distraction Phase

During distraction, the bone is gradually lengthened.

A commonly used rate is approximately 0.75 to 1 mm per day, although this may be increased, reduced or temporarily paused according to the patient's condition.

For example, femoral lengthening may progress at around 1 mm per day, while tibial lengthening may be performed more slowly.

An 8 cm lengthening therefore cannot happen immediately. The distraction itself requires several weeks, and commonly around three months when clinical pauses and adjustments are considered.

Regular X-rays are used to examine the quality of new bone formation.

5. Consolidation Phase

Once the target length has been reached, distraction stops.

The newly generated bone must then mature, harden and become strong enough to support increasing amounts of weight.

This is known as consolidation.

Patients continue rehabilitation and medical follow-up throughout this stage.

6. Return to Normal Activity

Weight-bearing and activity are gradually increased according to X-rays, bone strength, implant limitations and the surgeon's assessment.

Returning to unrestricted walking, running and sports takes considerably longer than reaching the desired number of centimeters.

7. Implant Removal

Internal nails are generally not removed immediately after lengthening.

When bone healing is sufficiently advanced, the surgeon may recommend removal at a later stage, often approximately one year or more after the original operation.

Timing varies according to the implant and individual healing.

What Methods Are Used for Limb Lengthening Surgery?

Modern limb lengthening can be performed using several systems.

The main categories are:

Internal Lengthening Nails

Internal lengthening nails are placed inside the medullary canal of the bone.

Systems such as PRECICE and FITBONE are examples of implantable lengthening technology.

The nail gradually extends through an externally controlled mechanism.

Because there are no external fixation pins crossing the skin during the distraction period, internal methods avoid many of the day-to-day difficulties associated with wearing an external frame.

However, internal nails are still orthopedic implants and have specific limitations.

Patients must respect weight-bearing restrictions because excessive loading before sufficient bone formation can damage the implant or bone.

Mechanical problems are also possible, which is why regular radiological monitoring remains essential.

LON — Lengthening Over Nail

LON stands for Lengthening Over Nail.

This technique combines an intramedullary nail inside the bone with an external fixation system during the lengthening stage.

The external device gradually creates the lengthening while the internal nail helps maintain stability.

After the required length has been achieved and the surgeon confirms that it is appropriate, the external fixator can be removed while the internal nail remains inside the bone during consolidation.

LON can provide effective lengthening but requires external pins during the distraction period.

As a result, careful pin-site hygiene is essential and patients must understand that external fixation generally creates more daily inconvenience and soft-tissue discomfort than a completely internal system.

External Fixation

Traditional limb lengthening can also be performed using an external fixator without an internal lengthening nail.

External fixation remains important in orthopedic reconstruction, especially for complex deformities, bone loss and certain medical cases.

However, for cosmetic stature lengthening, many patients and surgeons prefer internal or combined methods because of comfort, scarring and pin-site considerations.

The appropriate method should be selected according to the patient's anatomy and clinical needs rather than simply choosing the newest or most expensive technology.

Who May Be a Good Candidate for Cosmetic Limb Lengthening?

For cosmetic stature lengthening, candidates should generally have completed natural skeletal growth.

Closed growth plates can be confirmed on X-rays.

An appropriate candidate should also be able to understand that this is a lengthy orthopedic treatment requiring months of active participation.

Good candidates generally need:

• Appropriate bone quality

• Healthy joints

• Acceptable leg alignment

• Adequate muscle flexibility

• No active bone or soft-tissue infection

• Medical fitness for anesthesia and surgery

• Realistic expectations

• Willingness to attend regular X-rays and medical examinations

• Commitment to physiotherapy and daily exercises

• Ability to follow weight-bearing restrictions

Smoking and nicotine use are particularly important issues to discuss with the medical team because they can negatively affect bone and wound healing.

Body weight is also relevant because every implant has mechanical limits.

A surgeon may therefore recommend weight reduction or other preparation before surgery in some patients.

What Tests Are Needed Before Limb Lengthening Surgery?

The preoperative assessment should involve considerably more than simply measuring a patient's height.

Evaluation may include:

• Full-length standing X-rays

• Mechanical axis evaluation

• Femur and tibia measurements

• Joint examination

• Range-of-motion assessment

• Muscle and tendon flexibility testing

• Blood tests

• Medical and anesthesia assessment

• Review of previous orthopedic surgery or injuries

• Evaluation of current medications

• Bone-health testing when clinically necessary

Proportions should also be discussed for cosmetic patients.

The purpose of stature lengthening should not simply be to reach a predetermined number. The relationship between the femur, tibia, torso and total body height should be considered during planning.

Is Limb Lengthening Surgery Painful?

Limb lengthening is a major orthopedic procedure, so some postoperative pain should be expected.

The most significant surgical pain generally occurs during the first days following surgery and is managed with appropriate medication.

During the gradual lengthening period, patients may experience different types of discomfort rather than constant severe surgical pain.

These can include:

• Muscle tightness

• Stretching sensations

• Joint stiffness

• Muscle spasms

• Pain following physiotherapy

• Temporary nerve irritation

• Difficulty finding a comfortable sleeping position

Pain levels vary considerably between patients.

The rate of lengthening may sometimes need to be reduced or temporarily stopped if soft tissues are becoming excessively tight or neurological symptoms develop.

Pain should therefore be treated as clinical information, not something that a patient is expected to ignore in order to reach a height target.

Why Is Physiotherapy So Important?

Physiotherapy is one of the most important parts of limb lengthening treatment.

Bones can gradually regenerate inside the distraction gap, but muscles and other soft tissues also have to adapt to the increasing length of the limb.

Muscles may become tight more quickly than the bone lengthens.

Without adequate stretching and rehabilitation, this can reduce joint movement and potentially lead to contractures.

Physiotherapy focuses particularly on preserving:

• Knee extension

• Knee flexion

• Hip mobility

• Ankle movement

• Muscle flexibility

• Muscle strength

• Balance

• Safe walking technique

Patients are normally given exercises to perform outside formal physiotherapy sessions as well.

Consistency is essential.

Skipping rehabilitation because the X-rays show good bone formation can still create significant functional problems. Bone healing and soft-tissue flexibility must progress together.

Can You Walk During Limb Lengthening?

Mobility depends on the surgical technique, implant, stage of recovery, patient weight and bone formation.

Walking completely unsupported shortly after surgery is generally not appropriate.

Patients may use:

• A rollator walker

• Crutches

• A walking frame

• A wheelchair for longer distances

Internal lengthening nails have specific weight-bearing limits. Applying more force than the implant is designed to tolerate can result in bending or mechanical failure.

For this reason, patients should never increase weight-bearing simply because their pain has improved.

The amount of weight that can safely be placed on the legs should be determined by the surgeon after reviewing the implant and X-rays.

At Lengthening Turkey, patients are encouraged to remain mobile with appropriate assistance. A rollator is generally used for controlled walking and adaptation, while a wheelchair can be used for outdoor or longer-distance mobility when needed.

How Long Does Limb Lengthening Recovery Take?

There is an important difference between finishing the lengthening phase and finishing recovery.

Reaching the desired height does not mean the bone has fully healed.

For example, a patient undergoing several centimeters of lengthening may spend approximately two to three months in the distraction phase.

After distraction ends, the regenerate bone must continue consolidating.

Walking ability and weight-bearing then improve gradually as the bone becomes stronger.

Overall recovery is measured in months rather than weeks.

Many patients can return to desk-based activities considerably earlier than they can return to unrestricted physical activity.

Running, jumping, heavy weight training and contact sports require much more advanced bone healing and muscle recovery.

Individual timelines vary, and clearance for sports should always be based on radiological and functional assessment.

What Are the Risks of Limb Lengthening Surgery?

Every limb lengthening procedure carries risk.

Advances in orthopedic techniques and internal implants have changed the type and frequency of complications, but modern technology does not eliminate them.

Potential complications include:

• Infection

• Pin-site infection with external fixation

• Delayed bone formation

• Delayed union

• Nonunion

• Premature consolidation

• Joint stiffness

• Muscle contracture

• Nerve irritation or nerve injury

• Blood-vessel injury

• Malalignment

• Loss of range of motion

• Hardware bending, breakage or mechanical malfunction

• Fracture

• Deep vein thrombosis

• Pulmonary embolism

• Problems related to anesthesia

• Need for additional surgery

Some problems can be corrected by slowing or pausing distraction, changing physiotherapy, adjusting medication or modifying weight-bearing.

Others may require another surgical procedure.

For this reason, the quality of postoperative monitoring is just as important as the operation itself.

Patients considering surgery abroad should understand in advance who will monitor them, how frequently X-rays will be taken and how complications will be handled if they occur after returning home.

Warning Signs During Recovery

Patients should contact their medical team promptly if they develop concerning symptoms such as increasing redness or drainage around an incision or pin site, fever, sudden worsening of pain, unusual swelling, new numbness, weakness or loss of movement.

Sudden shortness of breath or chest pain requires urgent medical evaluation because these can be symptoms of a pulmonary embolism or another serious medical condition.

Patients should receive clear emergency instructions before leaving the hospital and again before travelling home.

Can Exercise Make an Adult Taller Without Surgery?

Exercise can improve posture, flexibility, muscular balance and the way a person carries their body.

It cannot, however, significantly lengthen the femur or tibia after the growth plates have closed.

Stretching, yoga, swimming and strengthening exercises may improve posture or flexibility, but they do not produce permanent several-centimeter increases in adult bone length.

Permanent lengthening of a mature long bone requires changing the bone itself.

This is why surgical stature lengthening is fundamentally different from posture exercises or programs advertised as natural height-increase methods.

What Happens to Muscles and Nerves When the Bone Gets Longer?

Bone is not the only structure affected during limb lengthening.

The surrounding muscles, nerves, blood vessels, skin, tendons and connective tissues must also adapt to the increasing length.

This is one of the reasons lengthening is performed gradually rather than creating the full correction during one operation.

Soft tissues have limits, however.

Muscles and tendons can become tight, and nerves may become irritated if distraction progresses faster than the body can tolerate.

Regular physical examinations are therefore necessary in addition to X-rays.

A good X-ray alone does not mean that lengthening should automatically continue.

Joint movement, muscle flexibility, nerve function and the patient's symptoms must all be considered.

Is the Height Increase Permanent?

Once new bone has fully consolidated, the additional bone length is permanent.

The body does not normally return the femur or tibia to its original length after the regenerate bone has matured.

However, permanent height gain should not be confused with immediate recovery.

The bone must first become sufficiently strong, muscles must regain strength and flexibility, and normal walking mechanics must be restored.

Limb Lengthening Surgery in Turkey

Turkey has become one of the countries considered by international patients seeking orthopedic and cosmetic limb lengthening treatment.

When researching limb lengthening surgery in Turkey, patients should compare considerably more than the advertised surgery price.

The complete treatment pathway is important.

Patients should investigate the surgeon's experience specifically with limb reconstruction and lengthening, the hospital where surgery will take place, the implant that will be used, frequency of physiotherapy, availability of radiological monitoring and the protocol for managing complications.

International patients should also consider where they will stay during the lengthening period.

Limb lengthening requires frequent follow-up and rehabilitation, so staying close to the treatment team can be more important than choosing accommodation based purely on tourism or city-center location.

Patients who plan to return to their home country before consolidation is complete should have a clear follow-up plan before leaving Turkey.

What Should You Ask Before Choosing a Limb Lengthening Center?

A serious limb lengthening program should be able to answer questions such as:

• Who will perform my surgery?

• Does the surgeon specialize in orthopedic limb reconstruction?

• Which hospital will the operation take place in?

• Which implant or technique will be used?

• Why is that method appropriate for me?

• What is the planned lengthening amount?

• Under what circumstances would lengthening be stopped early?

• How frequently will I receive physiotherapy?

• How often will X-rays be taken?

• Who reviews my X-rays?

• What are my weight-bearing restrictions?

• How are emergencies managed?

• What happens if my bone heals too slowly?

• What happens if an implant has a mechanical problem?

• What happens if I develop an infection?

• Who follows me after I return home?

• Is implant removal required?

• Which expenses are included and which are not?

Patients should be cautious about any provider that describes limb lengthening as simple, guarantees a specific number of centimeters or suggests that complications are impossible.

No responsible orthopedic program can guarantee how an individual person's bone and soft tissues will respond.

Limb Lengthening at Lengthening Turkey

At Lengthening Turkey, cosmetic limb lengthening treatment is planned around the patient's anatomy, mechanical alignment and recovery rather than height gain alone.

Before surgery, patients undergo orthopedic assessment and radiological planning, including evaluation of the mechanical axis of the legs.

Surgery is performed in a private hospital environment, followed by structured postoperative care and rehabilitation.

For patients remaining in Istanbul during the lengthening period, our program includes regular physiotherapy and repeated X-ray monitoring so that bone regeneration, alignment and progress can be reviewed throughout distraction.

Depending on the individual case, treatment may involve:

PRECICE internal lengthening nails

FITBONE internal lengthening nails

LON — Lengthening Over Nail

The appropriate method is discussed after medical evaluation.

Our general cosmetic planning limits are up to 8 cm for femoral lengthening and up to 6 cm for tibial lengthening, but individual limits can be lower.

Lengthening may be slowed, paused or stopped when medically necessary.

The primary objective is not simply reaching a number on a height chart. Maintaining alignment, joint mobility, healthy bone formation and long-term function is equally important.

Frequently Asked Questions About Limb Lengthening Surgery

What is another name for limb lengthening surgery?

The procedure may be called leg lengthening surgery, height surgery, height increase surgery, stature lengthening, cosmetic limb lengthening or distraction osteogenesis. Some of these terms specifically refer to cosmetic height increase, while distraction osteogenesis describes the underlying biological process.

Is limb lengthening major surgery?

Yes. Limb lengthening is major orthopedic surgery followed by a prolonged rehabilitation and bone-healing process. Patients should not approach it as a minor cosmetic procedure.

How quickly are the legs lengthened?

Lengthening commonly progresses at approximately 0.75 to 1 mm per day, although the rate can be changed according to bone formation, flexibility, symptoms and the surgeon's assessment.

Can I gain 8 cm from one surgery?

Approximately 8 cm can be an achievable femoral lengthening target for selected patients, but it should not be considered a guaranteed result. Some patients may need to stop at a shorter distance.

Can I gain 10 cm from only my femurs?

Lengthening should be limited by anatomy and soft-tissue tolerance rather than by a desired number alone. At Lengthening Turkey, the planned maximum for cosmetic femoral lengthening is generally 8 cm. Patients seeking substantially more height may need to consider a separate tibial lengthening strategy instead of excessively lengthening a single segment.

Can the femur and tibia both be lengthened?

Yes. In carefully selected patients, both segments can be lengthened as part of a quadrilateral treatment plan. This is a substantially larger orthopedic undertaking than single-segment lengthening and requires careful planning.

Does new bone really grow in the gap?

Yes. Gradual distraction stimulates the body to create regenerate bone between the separated bone segments. Over time, this tissue mineralizes and matures into stronger bone.

When does lengthening start after surgery?

There is normally a short latency period after the operation before distraction begins. In many cases, lengthening begins approximately one week after surgery, but the surgeon determines the timing.

Do I need physiotherapy every day?

Frequent physiotherapy and daily stretching are extremely important during stature lengthening. The exact schedule depends on the treatment program and individual condition.

Can I walk normally immediately after surgery?

No. Patients generally require a walker, crutches or another mobility aid. Weight-bearing limits depend on the implant, body weight, bone formation and stage of recovery.

When can I return to work?

Patients with desk-based jobs may return to some work earlier than patients whose jobs require prolonged standing, walking or physical labor. Remote work can often be easier during the distraction period. The timing should be discussed individually.

Can I run after limb lengthening?

Running can be possible after successful healing and rehabilitation, but it should only resume after the surgeon confirms that the bone has consolidated sufficiently and strength and joint movement have recovered.

Will the lengthened bone remain weaker forever?

After successful consolidation and remodeling, regenerate bone can mature substantially. Recovery is gradual, however, and the new bone should not be treated as fully healed during early consolidation.

Do internal nails need to be removed?

Many surgeons recommend removing internal lengthening nails after the bone has fully healed. Removal is usually performed much later than the original lengthening operation, often around one year or more afterwards depending on the implant and healing.

What is the best limb lengthening method?

There is no single method that is best for every patient.

Internal systems can avoid external pins during distraction and may provide greater day-to-day comfort, while LON and external fixation remain useful options in appropriate circumstances.

The decision should consider anatomy, treatment goal, bone size, body weight, medical history, budget and the surgeon's experience.

Is limb lengthening surgery safe?

Limb lengthening is an established orthopedic procedure, but it is not risk-free.

Outcomes depend on appropriate patient selection, surgical technique, controlled distraction, physiotherapy, regular imaging and early management of problems.

Patients should understand both the potential benefits and the possible complications before deciding whether surgery is appropriate for them.

Final Considerations

Limb lengthening surgery can permanently increase bone length, but successful treatment requires considerably more than the surgical procedure itself.

The complete process involves careful planning, surgery, gradual distraction, frequent rehabilitation, regular radiological monitoring and months of bone consolidation.

For patients considering cosmetic height increase, the most important question should not simply be:

"How many centimeters can I gain?"

It should also be:

"How can I achieve an appropriate amount of lengthening while protecting my joints, muscles, nerves, alignment and long-term ability to walk normally?"

A qualified orthopedic limb-lengthening surgeon should evaluate every patient individually before recommending a method or lengthening target.
